「これから治療が始まります」と言われたとき、いちばん不安なのは「いったい何が起きるんだろう？」「いつ終わるんだろう？」という、全体像が見えないことです。
具体的なメニューが決まるまでに、検査もたくさんあります。気づくと毎週のように病院に行っていて、「今やっていることは何ステップ目なんだっけ？」と分からなくなる方も多いです。
この記事では、診断から治療完了までの全体像を、6つのステップで整理します。
最初に地図を持っておくと、どんなに混乱しても自分が今どこにいるか分かるようになります。
まず大原則：治療は「順番」が大事
乳がん治療には、いくつかの選択肢があります。
乳がん治療の主な手段
- 手術（局所治療）
- 放射線治療（局所治療）
- ホルモン療法（全身治療）
- 抗がん剤治療（全身治療）
- 抗HER2療法（全身治療）
- 免疫療法（全身治療）
これらをどう組み合わせるか、そしてどの順番でやるかが、治療計画のポイントです。
順番のパターンは、大きく2通りあります。
治療ロードマップの6ステップ
ここからは、診断から完了までの流れを6ステップに分けて説明します。

ステップ1：診断と精密検査
「乳がんかもしれない」と告げられてから、確定診断と精密検査が並行して進みます。
この時期に行う主な検査
- マンモグラフィ・乳腺エコー（しこりの場所と大きさ）
- 針生検（組織を取って確定診断と病理検査）
- 造影MRI（広がりの精査）
- CT・骨シンチ・PET（遠隔転移の有無）
- 血液検査（腫瘍マーカー含む）
- 心電図・心エコー（治療前の体の状態確認）
このステップで、ステージとサブタイプが確定します。これが治療方針のベースになります。
ステップ2：治療方針の決定
すべての検査結果が揃ったら、主治医とこれからの治療方針を決めます。
ここで話し合うこと
- 手術が先か、抗がん剤が先か
- 手術の方法（部分切除 or 全摘、わきの手術の範囲）
- 乳房再建をどうするか
- 放射線治療の必要性
- 全身治療の組み合わせ
- 妊孕性温存の必要性（若い方）
- セカンドオピニオンを受けるか
迷ったらセカンドオピニオン。「主治医に悪い」と思う必要はないよ。むしろ、納得して治療に進むのは、主治医にとってもありがたいことです。
ステップ3：（必要に応じて）術前の全身治療
ステージⅢ・トリプルネガティブ・HER2陽性などで、術前に抗がん剤を行うことがあります。
術前抗がん剤（NAC）の特徴
- 期間：3〜6ヶ月程度
- 目的：しこりを小さくして手術しやすくする、薬の効き具合を確認する
- 通院：原則として外来通院（入院は不要）
- 副作用：脱毛・吐き気・倦怠感・骨髄抑制など
- ホルモン陽性：基本は手術が先（術前抗がん剤の効果が限定的）
このステップを行わない方は、ステップ2からステップ4に直行します。
ステップ4：手術
乳がん治療の中核となるステップです。
手術で決まること
- 乳房：部分切除 or 全摘
- わきのリンパ節：センチネルリンパ節生検 or 腋窩郭清
- 同時再建をするか、後日にするか
- 入院：3〜7日程度（病院によって異なる）
- 手術時間：1〜4時間程度
手術後、最終的な病理結果（pStage）が出るまで、約2週間かかります。
ステップ5：術後の追加治療
手術後の病理結果と当初の方針を踏まえて、必要な治療を行います。
術後に行いうる治療
- 放射線治療：部分切除なら基本的に行う（4〜6週間、平日毎日通院）
- ホルモン療法：ホルモン陽性なら5〜10年間の内服
- 抗がん剤：必要に応じて3〜6ヶ月
- 抗HER2療法：HER2陽性なら1年間
- ベージニオなど追加薬：高リスクの方

ステップ6：定期検査と長期フォロー
すべての積極的な治療が終わると、定期的な経過観察に入ります。
定期検査の内容
- 半年〜1年ごとの診察と血液検査
- 1年ごとのマンモグラフィ・エコー
- 必要に応じてCT・骨シンチなど
- 副作用や後遺症の管理
- 期間：基本的に10年間
治療が終わっても、定期検査は続きます。「もう病院に来なくていいよ」と言われるのは、診断から10年後くらい。気が遠くなるかもしれないけど、それくらい長く一緒に伴走するつもりだから、安心してね。
いつから普通の生活に戻れる？
これは個人差が大きいですが、目安はこんな感じです。
生活の戻り方の目安
- 手術直後：1〜2週間で日常生活はほぼ元通り
- 仕事復帰：手術のみなら1ヶ月、抗がん剤併用なら治療終了後
- 運動：手術後1ヶ月から軽い運動、3ヶ月で本格的に
- 旅行：抗がん剤中は控えめに、終了後は徐々に再開
- 通院頻度：抗がん剤中は週1〜3週ごと、ホルモン療法中は3〜6ヶ月ごと

ここまでの流れを、もう一度俯瞰してみましょう。

「治療が終わるまで〇年」と言うのは難しいですが、ホルモン療法を含めた完全な完了までは、5〜10年というイメージです。
ただし、ステップ5の途中からはほぼ普通の生活ができますし、毎日が治療一色になるわけではありません。
